What to bring

List medicine names, strengths and how often you take them. Include supplements and any medicines recently stopped.

Write down the main questions you want answered, plus any companion, communication or accessibility support you may need.
Identification and registration details: bring these only when the clinic team specifically asks for them. Do not send identity documents through ordinary messages unless an approved secure process is provided.
